Basic Hair Follicle Anatomy

Hair follicles are associated with other organs within the skin. As the follicles grow, they connect with developing sebaceous glands, and erector pili muscles. These structures form the folliculosebaceous apocrine.

The infundibulum is the portion of the hair in the follicle, extending from the surface down to the papilla matrix in the follicle.

The isthmus of the hair then extends from the gland entrance to the to the sebaceous oil gland. The stem extends from the oil gland to the erector pili muscle connection.

The bulb of the hair follicle is deep to the attachment of the erector pili muscle and is the site of the papilla matrix. This is the primary site responsible for hair growth and development. Human adult hair has 3 distinct phases of development. The first bein anagen, then catagen, and finally telogen. As hairs progress through these phases, they are affected differently by treatment with laser or pulsed light hair removal.

Anagen is the phase in which active hair growth occurs. Papilla matrix cells are actively producing cells that will make up the emerging hair.

In catagen (the slowing phase), matrix cells cornify and the bulb begins to atrophy, causing the hair follicle to shrink.

Telogen is the final phase of the hair follicle in which the follicle rests. Growth has stopped, and the follicle becomes inactive. During telogen, the hair is released from its attachment to the skin and eventually is shed from the skin. The process begins again in anagen to form a new hair.

The length of time spent in each phase depends upon the location of the hair, type of hair and other hormonal factors. Scalp hair can be anagen for as many as ten years. Body hair normally is active for about 4 weeks.

Normally 90% of hairs in a given body area can be in the anagen phase at any given time. This becomes important because only hairs in the anagen phase of development are susceptible to injury from lasers or pulsed light treatment. For this reason multiple treatments are necessary for effective reduction of hair growth.
